A Brazilian buttock lift (BBL) is also known as fat injection to the buttock. This post will describe the process of increasing the buttocks’ size and shape using one’s own fat. The final shape and appearance of the buttock depends on your anatomy, precise fat injection but also shaping the buttock with selective liposuction of the buttock and the tissue around it.
The process for performing a BBL is the same regardless of the patient’s weight.
This is truly a win-win situation. Unwanted areas of diet-resistant fat are liposculpted and not only enhance the overall body’s contour but provide viable fat cells for grafting via precise injections into the buttock to enhance size and shape. Specific liposculture is completed around the buttock to attain the shape of the buttocks the patient desires.
There are various ways to separate the viable and graftable fat cells from tumescent fluid (a fluid injected prior to liposuction that contains epinephrine and small amounts of local anesthetic agent), blood, disrupted fat cells and other cellular debris. These techniques include but are not limited to centrifuge, filtration and extraction.
The purified fat is now ready to inject. Based on the patient’s desires of buttock shape and size, precise injections are placed into the buttock through very small hidden access sites. Once the fat is incorporated into the buttock, it becomes permanent living tissue. The fat that doesn’t survive is broken down, dissolves and is eliminated from your body using your own body’s natural and safe processes.
The amount of fat injected depends on how much fat is available to harvest. One can try to gain weight prior to the procedure in the hope that the areas you gained the weight can be liposuctioned. The caveat is not all fat gained can be liposuctioned — fat deposited on your face, breast, internal organs, etc. Also losing or gaining weight will always change your body contour. Optimally, you should be at your goal weight. Liposuction can refine and sculpt the areas of diet-resistant fat in relationship to your baseline weight of your body.
Should you lose weight after the BBL, some of the fat volume may be lost in the buttock injected areas; thus, the size of the buttock may be reduced.

The amount of fat permanently incorporated into the buttock depends on several factors.
- The gentle technique of harvesting the fat (providing the least amount of damage/trauma to the fat cells)
- How the fat is injected
- The recipient area (good blood supply, lack of previous scars, etc.)
- Postoperative care
- A boney pelvic structure
- A muscular buttock shape
- The elastic (“stretchability”) quality of your skin
