Tummy tuck is also known as Abdominoplasty. When liposuction is added as part of the standard procedure, it is referred to as Lipo-Abdominoplasty. There are variations of the standard or basic tummy tuck referred to as a mini or extended abdominoplasty. Thus tummy tucks of any variations has a high satisfaction rate. According to RealSelf 97% of patients voted “its worth it”. However just as in living “life happens” and untoward events can happen despite careful planning, patient compliance and “doing everything correctly”. Fortunately most are minor challenges and rarely some can be serious. The purpose of this writing is to present on how we as patients and Plastic Surgeons can take steps in minimizing and prevention of such untoward events.

WHAT IS A TUMMY TUCK OR ABDOMINOPLASTY PROCEDURE?

First to better understand the potential challenges one needs to understand the basics of the tummy tuck or abdominoplasty. The classic or standard tummy tuck (abdominoplasty) procedure usually includes:

  • removing lower abdominal excess skin and fat, resulting in a smoother and flatter contour
  • reapproximating separated central abdominal muscles (diastasis rectus)
  • intraoperative injection of long acting local anesthesia.
  • I also insert a non narcotic pain pump that automatically drips in long acting local pain medication often precluding use of post operative narcotics for pain relief.

Commonly associated procedures:

  • liposuction of the flanks
  • repair of any hernias (umbilical and inguinal if present)
  • insertion of temporary (usually 3 days) non narcotic pain pump

Common risks/complications & Preventative Steps

1. Bleeding, hematoma or fluid collection.

PREVENTION:

  • no Aspirin, Celebrex, Aleve, Motrin mega vitamin E,or a diet high in omega 3’s 2 weeks prior to surgery
  • all supplements
  • any prescription medications (check with your doctor on how to compensate for)to thin your blood at least 2 weeks prior to surgery.)
  • If you do have any headaches or pain I prefer plain Tylenol.

2. Blood clots, deep vein thrombosis (DVT) and pumonary embolus.

PREVENTION:

  • no alcohol night before (can dehydrate)
  • fleets enema night before. Removes hard feces that may occlude internal abdominal veins when muscles re/approximated thus increasing infra abdominal pressure.
  • temporary and calculated administration of anti coagulant by injection just prior to surgery
  • use sequential compression devices both pre, intra and postoperatively. In fact we send all abdominoplasty patients home with portable and rechargeable SCD units that are be used for the first two weeks after surgery.
  • Early walking, calf compressions and ankleflexing is associated with prevention of DVT’s

3. Pneumonia/wound Infection

PREVENTION:

  • 3 days before surgery: take a shower morning and evening with antiseptic solutions and place mucopurien (prescription only) topical antibiotic in the nasal opening, umbilicus (tummy button) and external ear canal.
  • IV antibiotics will be given preoperatively just before surgery
  • all patients receive a device called a incentive spirometer that encourages deep breathing.
  • It is also important for patients to start walking as soon as possible after surgery.

4. Anesthesia.

PREVENTION:

  • Age/significant medical history
  • if over age 45 or any significant health issues a preoperative evaluation and lab work will required your by primary care physician.
  • Where Surgery is performed: a fully accredited and certified Ambulatory Surgical Center such as Riverside Outpatient Surgical Institute (ROSI)

5. Skin-vascular insufficiency resulting in skin incision separation and possible skin loss.

PREVENTION:

  • No smoking or second hand smoking for two before surgery and 1 month after surgery.
  • For the first four days walking in the bent over position(we loan you a walker for the first 4-7 days).
  • Sleeping in the lawn chair position or fetal/curled position for the first 4 days after surgery.

6. Temporary Urinary retention.

The general anesthesia side affects is also puts the nerves to the bladder asleep resulting in a high incidence of urinary retention in the first several hours after surgery after abdominal surgery

PREVENTION:

  • The general anesthesia also puts the nerves to the bladder asleep resulting in a high incidence of urinary retention in the first several hours after surgery. Thus, a bladder catheter is inserted (while your asleep) and removed 6-8 hours after surgery.

7. Fluid accumulation (seroma). This a a collection of swelling fluid that can accumulate unerring the skin and above the muscle.

PREVENTION:

  • During surgery “quilting sutures” are placed as well as temporary drainage tubes that are removed when drainage is resolved.

8. Poor wound healing, asymmetry of the incision and unfavorable scar formation

PREVENTION:
  • NO smoking! Smoking and second hand smoking is the number one problem of poor wound healing and unsightly wide scars.

  • Some times the incision heals in a unanticipated way and can be revised under local anesthesia if necessary

9. Skin discoloration and/or prolonged swelling.

PREVENTION:

  • individual genetics plays a large role in the color of the final scar and amount of swelling.
  • We strongly believe in using sun block over all unctions and avoidance of direct sunlight or tanning booths for the first 6 months over all incisions.
  • A compression garment like “spanks” usually removes abnormal and persistent swelling.

10. Recurrent looseness of skin.

PREVENTION:

  • As we age we naturally loose collagen and elastic fibers of the skin. The amount is genetically related. However after pregnancy or massive weight loss the skin actually permanently decrease it “viscoelastic properties which means the skin will again stretch. Anticipating and compensating this we perform the surgery in the “lawn chair position” and actually have you walk and sleep in the hunched over position for the first 4 days after surgery.

11. Fatty tissue found deep in the skin might die (fat necrosis.)

This is a rare and usually occurrence and included for completeness.

PREVENTION:

  • Gentle and precise liposuction and skin excision is always completed. However if a firm lump does not resolve injections are first used to dissolve the lump and rarely need a secondary procedure.

12. Suboptimal aesthetic result, asymmetric scars and “dog ears” resulting in need of a possible revisional surgery.

PREVENTION:

  • As in life, even with careful and precise surgery, unusual healing occasionally occurs. Fortunately most revisions when needed are completed using only local anesthesia in the minor room suite.

WHAT DO A SLIT EAR LOBE, A EAR LOBE HOLE AFTER GAUGE EAR RINGS OR LARGE SIZED EAR LOBES HAVE IN COMMON?


All have one element in common: a unwanted and noticeable appearance of the ear lobe.

  • Slit ear lobe: caused by the weight of ear rings and or having a ear ring pulled down and dividing the earlobe skin. This is a spectrum and before a complete split of the ear lobe the ear lobe piercing hole becomes elongated and can result in the in ability to wear stud earrings or noticeable asymmetrical ear ring placement/location.
  • Gauge ear lobe holes: caused by the insertion of serially larger earlobe inserts. Often the resultant expanded ear lobe hole will not retract and when one discontinues the use of the ear lobe spacers the hole remains leaving a undesirable large hole and odd appearance.
  • Large ear lobes: one can be born with with large ear lobes, ear lobes that have increased, elongated with time and age or because excessive in size because the weight of “clip on” ear rings stretching the earlobe.

EAR LOBE REPAIR TREATMENT OPTIONS

Basically, depending on the patients desire there are several techniques to repair each of the above ear lobe deformities.

EAR LOBE REPAIR CANDIDATES

Since most of these acquired or congenital (born with) deformities are repaired under just local infiltrative anesthesia (a general anesthesia however can be a option) anyone who has general good health and desires cosmetic improvement of the ear lobe is a candidate.

HOW AN EAR LOBE IS REPAIRED

The results are permanent and as long as long as no additional trauma or heavy ear rings are worn there is rarely needed secondary surgery.

Most patients return the following day to work r after the normal post surgical swelling resolves in several days. External non absorbable sutures are removed in 5-7 days.

BREAST AUGMENTATION, IMPLANT OR BOOB JOB RECOVERY

Recovery after boob job surgery is often patient dependent but in general the effects of anesthesia are gone after 12-24 hours. Inter-operative local anesthesia injections last 12-24 hours.  If a non narcotic pain pump had been inserted pain is a minimum and usually non prescription pain relievers are only necessary. Thus, driving and returning to sedentary work can be accomplished in 24 hours after surgery. Patients usually will return to non physically demanding jobs in 2-3 days and 1-2 weeks for occupational physically requiring upper body activity.

TIME TO HEAL

Depending on how you define “healing in general is 6-8 weeks to return to full or all pre-breast implant or “boob job” physical activity without restrictions. However the final breast augmentation results may take several months.

WHAT ARE THE HEALING PHASES?

There are several phases:

The Acute Phase

This is usually between 2-3 days when it is expected to have the most pain because the early stretching of the muscle (if Implant is placed under the muscle). Most patients return to non physical demanding jobs during this phase.

Early Phase

This usually the first 5-6 weeks at which time implant has now “seated”, “fluffed” or “dropped”. Silicone gel strips are started to minimize the external scar appearance. Progressive upper body exercise is permitted as long as there is no chest pain.

Mid Phase

This time period is the first 6 months. Full exercise with out restrictions is permitted. It is expected for the breasts to becomes progressively more naturally feeling, the absorb-able sutures have absorbed, the incision is softening/fading and the breast implants have “become you”.

Late Phase

Is categorized as the 6-12 month period. The incisions are often soft, supple and have lightened. The breasts have the look and feel you desire. If a revision is needed it is usually performed during this time period.

Before & After

HOW TO HELP THE HEALING PHASES

Plastic Surgeon Dr. Hardesty is a  firm believer in early and often massage or displacement exercises to hasten the desired location and permanent positioning of the breast implants. Just as size is a personal decision, so is the position of the implant in relation to the chest wall.

Without displacement exercises the natural action of submuscular placement of breast implants is to push the implants “up and out” which is most often not the desired position of the breast implants. I thus recommend a “5-5-5” program for the first 5 weeks after surgery which is:

  • 5 min a time
  • 5 times a day
  • 5 weeks

Where the patient massages and presses the implant centrally for more cleavage, down If to high or if asymmetry asymmetrical pushing to create symmetry. We have different straps, splints and additional displacement exercises that can be recommended or performed during “early phase” time period. During this period it is recommended to wear a non support or stretch bra.

Once the desired location and appearance is achieved I recommend a good supportive bra during regular activity and “double” sports bra during high impact activity as a preventative measure to prevent premature sagging (ptosis) of the Now enlarged bra.

HOW LONG DO BREAST AUGMENTATION RESULTS LAST?

Once the breast implants are “seated”, “fluffed” or “dropped” the position is made permanent by placement of a underwire bra. The “bigger you go… the faster you will drop” or sag is a natural fact. Thus good support is mandatory during usually activity and extra support during vigorous or high impact exercise. Since implants have technically improved (shell durability and cohesive “gummy bear” silicone gel) so much there is no longer a recommended 10 year replacement.

WHAT WILL NOT BE TREATED?

Both internal (capsular contracture) and external scarring is predominately a genetic and individual phenomena. Despite delicate and precise surgical technique, precautionary treatments and compliance with post operative instructions a small minority of patients will need revisions surgery during the first year.

SURGICAL OPTIONS AFTER MASSIVE WEIGHT LOSS

WHO IS A CANDIDATE FOR SURGERY AFTER MASSIVE WEIGHT LOSS?

Plastic surgery procedures after massive weight loss can be life changing. Unfortunately while staying a more normal body weight many are left with diet resistant fat areas, loose and hanging skin that precludes wearing certain clothes and even effectively exercising. In addition the psychological stigma often remains and is often a unpleasant reminder of pre weight loss appearance.

Prospective patients after massive weigh loss may have additional medical issues that need to be investigated prior to surgery especially if the weight loss is from a surgical bariatric procedure. After bariatric surgery the patient has been surgically purposely placed in a “malasorbation state” that may result in a malnourished state that will require correction before surgery.

It is recommended to have through evaluation that includes but limited to:

  1. Having a stable and desired weight for 6 months
  2. Normal hemoglobin/hematocrit (oxygen carting blood component)
  3. Normal albumin, protein, and transferritin (required for normal wound healing )
  4. Normal electrolytes (basic blood serum chemicals)
  5. Electrocardiogram (EKG)
  6. A evaluation and clearance by the primary care or equivalent physician

PREPARATION FOR PLASTIC SURGERY AFTER MASSIVE WEIGHT LOSS

  1. A diet to include at least 100 grams of protein per day before and after surgery to provide the needed healing components.
  2. Multi vitamin to start daily 1 month prior to surgery and for at least a month after surgery that are required as cofactors for normal healing.

WHAT CAN CAUSE DIFFICULTY IN BREATHING?

There are several common causes of nasal obstruction:

  • Allergies
  • Deviated septum (the portion separating the left and right nasal passages). This can be congenital (born with it) or acquired (injury or traumatic)
  • Large turbinates which are a set of 3 paired out pouches that function to humidify the air we breath). These can be congenitally large, become large to compensate for a deviated septum or because of chronic allergies.
  • Nasal polyps are usually non cancerous growths that grow and by their size obstruct the airway.
  • Combination of the above

WHAT WILL SURGERY HELP?

Deviated Septum: If the septum is crooked and is causing airway obstruction a septoplasty can be performed to improve the nasal passage(s) opening and thus airflow resulting in better airflow.

Hypertrophic (large) Turbinates: If the turbinates are large and causing decreased air flow they can be surgical reduced while still slowing them to function.

Nasal Polyps: If there polyps that are obstructing air flow through the nasal passages likewise they can be removed.

WHO IS A CANDIDATE FOR INTERNAL NASAL SURGERY?

Anyone has airway obstruction, is a moderately good health and desires and improvement.

HOW IS A SEPTOPLASTY, TURBINATE REDUCTION OR POLYPECTOMY PERFORMED?

Septoplasty

A septoplasty is performed using only internal nasal incisions on the nasal mucosa (the lining tissue of the septum) gently and precisely lifting the nasal mucosal off of the septum and either removing the obstructing portion of the septum or straightening the septum.

Turbinate reduction

Turbinate reduction is usually performed by:

  • Making an incision over the mucosal of the turbinates removing the underlying protruding and obstructing turbinate bone.
  • Reducing the turbinate by amputating it at his base -or using a technique of “out fractioning” the turbinate bone which collapses the turbinate bone and then shrinking the mucosal with a electrocautery.

Nasal polyps

Nasal polyps are removed using only internal incisions and the polyps are usually removed at their base using electric cautery or a snare with electrocautery.

HOW LONG WILL THE RESULTS LAST?

Deviated Septum Results

Once the deviated septum and is repaired and healed the septoplasty should last a lifetime unless additional trauma occurs.

Turbinate Reduction Results

Turbinate reduction is a permanent surgery. It is only very rare cases that either the turbinate boney grows back or there can be hypertrophy of the turbinates nasal mucosa. This is unusual and may be needto be addressed if it again causes nasal airway obstruction.

Nasal Polyps Results

Nasal polyps usually do not grow back. However, once you’ve developed a nasa polyp, you may grow new polyps which may cause additional symptoms of obstruction. Thus you may need to have these additional polyps removed.

HOW LONG DOES RECOVERY TAKE?

Septoplasty Recovery

The usual septal recovery is 7 to 10 days. The reason is that often internal splints (Doyle splints) are placed and sutured to prevent movement. Occasionally and rarely nasal packing is used. Often the patient returns to work in 2 to 3 days depending on how they feel. The splints are removed usually at around the 10th day.

Turbinate and Polypectomy Recovery

Turbinate and polypectomy recovery is usually straight forward and those with non physical or non strenuous work the recovery is only a few days.

ALTERNATIVES TO SEPTOPLASTY, TURBINATE REDUCTION OR POLYPECTOMY ?

Unfortunately when there is a mechanical obstruction there are very few non-surgical options. Most patients have tried using antihistamines, nasal mucosal shrinkage sprays such as Afrin nasal decongestants. However, patients can develop resistance to such medications. when the symptoms are not relieved patients often then consult a Plastic Surgeon for remedies.

WHAT WILL SEPTOPLASTY, TURBINATE REDUCTION OR POLYPECTOMY NOT HELP?

If the septum is not crooked, the turbinates are not enlarged or the polyps do not cause airway obstruction then these types of surgery will not help in improving breathing.

Allergies are the most common common cause of non mechanical airway obstruction. If the airway obstruction is caused by environmental allergies or other substances it can often be treated medically by a allergist specialist

CAN I COMBINE WITH OTHER PROCEDURES?
